VSP was founded in 1955 by a group of optometrists who wanted to provide access to affordable, high-quality eye care to the world. When you choose VSP, you can be confident you’re choosing a company that puts your vision care needs first.
VSP is the only national not-for-profit vision care company. We reinvest in our members and the community through the advancement of optometry, eyewear donations, and mobile clinics to help connect people in need with a local VSP network doctor.
VSP network doctors provide the most comprehensive eye exam possible—a WellVision Exam. Through this exam, doctors get a better view of your eyes and look for eye health problems such as glaucoma, and diabetic eye diseases while looking for signs of serious health conditions like high blood pressure.

VSP members who enter personal information should know all communication between their computer and VSP's Web servers are encrypted using Transport Layer Security (TLS 1.2) technology. Personal information collected and how it is used:
VSP collects anonymous, non-personal information about users of this site through temporary session cookies. Cookies are small, alphanumeric identifiers that are transferred to a computer's hard drive through the user's Web browser, which enables VSP's systems to recognize the browser and provide personalized features. Session cookies expire when you close your browser.
VSP will collect data about the user's activities that do not personally or directly identify the user when visiting our website. If the User allow his/her minor child, or a child for whom User is a legal guardian (a "Minor"), to access and use the features, User agree that he/she will be solely responsible for: (a) the online conduct of such Minor; (b) monitoring such Minor's access to and use of the Service; and (c) the consequences of any use.
VSP responds to emails, electronic messages, and other communications as soon as reasonably practical. VSP's response time shall not exceed any maximum time period that may be specified by law.

You agree that Abusive Use of the Site, as defined above, causes damage and harm to VSP in the form of, among other things, impaired goodwill and increased expenses associated with responding to Abusive Use of the Site. You further agree that monetary damages for Abusive Use of the Site are difficult to ascertain and that proof of monetary damages for Abusive Use would be costly and inconvenient to calculate. Accordingly, you agree that injunctive or other equitable relief is warranted for Abusive Use. Therefore, you agree that if you, or others act in concert with you, alone or collectively request more than 100 pages of content from the Site in any twenty-four hour period, you, and those acting in concert with you, may be held jointly and severally liable for reasonable attorney's fees, expert witness fees and costs associated with injunctive or other equitable relief.

described in this notice. If it becomes necessary to disclose any of your Protected Health
Information for other reasons, VSP will request your written authorization. VSP will obtain your
authorization for any sale of Protected Health Information, or to use or disclose your Protected
Health Information for marketing.
Revoking Authorization
If you provide written authorization, you may revoke it at any time in writing, except to the
extent that VSP has relied upon the authorization prior to its being revoked.

VSP may disclose your Protected Health Information to a family member, friend, or other person

Review Your Protected Health Information
You have a right to inspect and obtain a copy of your Protected Health Information.
Important : If you feel your Protected Health Information is incomplete or incorrect, you have the right to request that it be amended.
Request to Restrict Your Protected Health Information
You can request restrictions on the use and disclosure of your Protected Health Information. VSP
is not required to agree to a requested restriction.
Example : If a restriction request prevents us from providing service to you or from performing payment related functions, we will not be able to agree to the request.
Confidential Communication
When necessary, VSP may seek to contact you by calling you at your home or by sending mailings
containing your Protected Health Information to your home. If you feel that such communications
could compromise your safety, you may request in writing an alternate communication method and/or
location.
Important : VSP may require that a request contain a statement that disclosure of all or part of the information to which the request pertains could endanger the individual, and VSP may, if and to the extent that applicable law allows, request payment for this service.
Examples : The patient may decide, for his or her safety, to have correspondence containing his or her Protected Health Information sent somewhere other than to his or her home, or to have the information sent via fax rather than mailed.
Accounting of Disclosures
If a disclosure of your Protected Health Information was made for a reason other than treatment,
payment or health care operations, you have a right to receive an accounting of the disclosure.
Important : If the disclosure was made to you, VSP will not provide an accounting.

You can view and print a copy of this Notice of Privacy Practices through vsp.com. You may also
request a copy from your Benefit Administrator, or you may request a paper copy from VSP
.
Complaints
If you believe that your privacy rights have been violated, you may submit a complaint to VSP or
to the U.S. Secretary of Health and Human Services at any time. VSP will not retaliate against you
